Important Notice to Consumers regarding Driving Behavior Reports

Verisk no longer receives driving behavior data from automakers to generate Driving Behavior Data History Reports. Verisk no longer provides Driving Behavior Data History Reports to insurers. If you’re interested in receiving a copy of your Driving Behavior Data History Report, please click on the link at the bottom of the page. The driving behavior related data Verisk can offer you will vary by auto manufacturer. See table below.

You can contact your automaker directly for additional information regarding your participation in their driver feedback or related programs, including information regarding enrollment and unenrollment activity.

Please fill out the below forms by clicking on the appropriate blue box or download and print the appropriate form. Once completed, submit to FCRA-NPC@verisk.com, or fax to 800-955-2422 or mail to: Verisk Underwriting Solutions Consumer Inquiry Center P.O. BOX 5404 Mt. Laurel, NJ 08054.

To dispute information on a claim record or Insurance Policy record or to obtain a copy of a report resulting from an adverse action notice by an insurance company, consumers can call the Consumer Inquiry Center at 1-800-709-8842. Telephone requests for disputes are accepted Monday through Friday from 9 AM to 5 PM EST. During non-business hours and on weekends and holidays, you may leave a message with your name and telephone number.

**To request a free FACTA (Fair Accurate Credit Transactions Act of 2003) report call the Verisk Consumer Report Request Line at 1-800-627-3487. The Verisk Consumer Report Request Line is available Monday through Friday from 9 a.m. to 5 p.m., Eastern time. During non-business hours and on weekends and holidays, you may leave a message with your name and telephone number. A representative will return your call on the next business day. Note: We will send the reports by mail only.
